10. Issuance of a duplicate document on education is carried out in compliance with the following procedure:

b) a duplicate of the certificate of basic general education is issued within three days; a duplicate of the certificate of secondary (complete) general education - after a month;
c) when issuing a duplicate, in the column "Receipt for receipt" it is written that the duplicate was issued to replace the lost original, indicating its number and date of issue;
d) a duplicate is signed by the head of the educational institution, the deputy head for educational work and teachers (at least three).
11. Duplicate documents on education are issued on blank forms valid at the time of issue.
12. In the event of reorganization of a general education institution, the issuance of duplicate documents on education is carried out on the recommendation of the municipal education authority by the educational institution - the successor of the reorganized institution, in which the archive of the reorganized general education institution is stored; the document must be signed by the head, his deputy for educational work, three members of the pedagogical council of the general education institution and sealed with the seal of the general education institution.
In the event of liquidation of a general educational institution, the issuance of duplicates is carried out by the municipal education authority according to archival data and sealed with the seal of the relevant education authority.
13.
